Background: The Sepsis Alert was introduced as a triage model in the emergency department of Malmö university hospital in 2012 to expediate and improve the care of patients with a suspected serious infection. The alert was implemented in the rest of Skåne in 2016 and is now being adopted in all Swedish emergency department as part of a standardised course of care. The Sepsis Alert has never been evaluated in randomised controlled trial.

Purpose: To evaluate and improve the Sepsis Alert.

Method: In co-operation with Professor Jens-Ulrik Jensen of Copenhagen University, we plan to perform three consecutive studies. First, a retrospective review of medical journal data from Swedish and Danish emergency department visits with the aim of improving the criteria for the Sepsis Alert through predictive modelling. Secondly, a prospective study in Sweden to validate the new criteria. Thirdly, a cluster-randomised controlled multicentre trial to evaluate whether the introduction of the old or new Sepsis Alert leads to a better 28-day survival compared to no Sepsis Alert in emergency departments in Denmark and Sweden (no control group in Sweden).

Implications: We expect the results from the randomised trial to be decisive as to whether Denmark or other countries should implement a Sepsis Alert triage model, or whether Sweden should re-assess this resource-demanding initiative.
