The purpose of this exploratory and multi scientific research is to develop a cultural understanding of barriers, facilitators and local leadership during the systematic implementation of hospital based home care (HBHC) for children newly diagnosed with diabetes type 1 in two organizational and socioeconomic contexts (Malmö and Lund).

The purpose of this exploratory and multi scientific research is to develop a cultural understanding of barriers, facilitators and local leadership during the systematic implementation of hospital based home care (HBHC) for children newly diagnosed with diabetes type 1 in two organizational and socioeconomic contexts (Malmö and Lund). We aim to contribute to the discussion of barriers and facilitators to implementation through an approach to culture that deviates from much of the earlier research concerning culture an implementation. The goal is to increase the understanding and knowledge of the decision-making processes, the contextual barriers and facilitators of change and the role of the local leadership. The contextual effects on the implementation practise will be explored and evaluated at cultural and historical levels by the use of interviews, observations, and fieldwork, contributing to the overall development of theoretic models for handling contextual complexity in implementation processes. The safety and feasibility of HBHC will be evaluated in terms of patient and family outcomes and health care costs.
