Replacing hips and knees with artificial implants are today very successful interventions when joints are worn out or damaged. Still, however, some of these prostheses have to be replaced due to, for example wear, misplacement, or loosening. Reoperations are often demanding surgery both for the patient and surgeon, and a cost burden for the society. In order to optimize the surgery, and minimize the complication risks, the operations are normally pre-planned on standard radiographs to have a better idea of what implants should be used and how they should be positioned. Now, 3D-planning, based on Computer Tomography (CT), has become an emerging option and we have previously shown that this is possible to do on low-dose CT-scans. Furthermore, based on the preoperative planning, surgical navigation during the operation can make sure the implants are positioned according to plan. The technique is still under development and early available systems are time-consuming and complicated. We have during several years been involved in the development of a Swedish system for 3D-planning and surgical navigation which seems fast, easy to use and very accurate, but further scientific evaluation and development is needed. We believe this new software can also be used for implant migration analysis, for which we have for many years been using the current gold standard; Radiostereometric Analysis (RSA). If the same analysis can be based on low-dose CT-scans and Artificial intelligence (Al) solutions instead it will make it easier and the technique more available.
